Simone Biles has ADHD. She has taken medication for it since childhood. She is also the most decorated gymnast in history — male or female — with 41 combined Olympic and World Championship medals. The distance between those two facts is not as large as it looks.
Was Simone Biles actually diagnosed with ADHD
Yes. Biles was diagnosed with ADHD as a child, and she has spoken about it publicly on multiple occasions. In her 2016 autobiography Courage to Soar, she wrote about the diagnosis directly. Her teachers had noticed her drifting in class, distracted by whatever happened to be more interesting than the lesson. Her parents sought answers. The diagnosis came back ADHD.
"Given how hyper and energetic I'd always been as a little kid, no one was entirely surprised," Biles has said.
The public disclosure, however, was not her choice. In September 2016, weeks after Biles won four gold medals at the Rio Olympics, a Russian hacking group calling itself Fancy Bears breached the World Anti-Doping Agency database and published the confidential medical records of several American athletes — Biles among them. The leaked files revealed that she had a Therapeutic Use Exemption for methylphenidate, a stimulant medication used to treat ADHD that appears on WADA's prohibited substances list.
The intent was to discredit her. The effect was the opposite.
Biles responded with a statement that has since become one of the most widely quoted declarations about ADHD in sport: "I have ADHD and I have taken medicine for it since I was a kid. Having ADHD, and taking medicine for it, is nothing to be ashamed of and nothing that I'm afraid to let people know."
USA Gymnastics confirmed that Biles had submitted the proper paperwork and been approved. She had broken no rules. What the hackers revealed was not a scandal. It was a medical condition being treated by a doctor.
From foster care to the gym floor
The story of how Biles arrived at gymnastics begins before the diagnosis.
Simone Arianne Biles was born on 14 March 1997 in Columbus, Ohio. Her biological mother struggled with addiction, and the children were placed in foster care when Simone was three. She and her younger sister Adria were eventually adopted by their maternal grandparents, Ron and Nellie Biles, and relocated to Spring, Texas, when Simone was six.
On a daycare field trip to Bannon's Gymnastix in Houston, a coach noticed Biles mimicking the older gymnasts with startling natural ability. She was invited to train. By fourteen, she had made the US junior national team. By sixteen, she won her first World Championship all-around title.
The timeline illustrates a pattern that ADHD researchers have documented repeatedly. A 2019 narrative review by Han and colleagues in the British Journal of Sports Medicine estimated that ADHD prevalence among elite athletes may be 7 to 8 per cent — higher than the roughly 5 per cent rate in the general population. Children with ADHD gravitate toward sport because of what the researchers called the "positive reinforcing and attentional activating effects of physical activity." The gym gives the ADHD brain what the classroom cannot: immediate feedback, physical engagement, and a reason for the nervous system to pay attention.
Biles found that reason at six. She has never let go of it.
How ADHD works in a body that flies
To understand how Biles competes at the level she does, you need to understand what ADHD actually changes in the brain — and why some of those changes, in the right environment, become advantages.
ADHD involves measurable differences in dopamine regulation and prefrontal cortex activation. Russell Barkley, one of the foremost ADHD researchers, describes it as fundamentally a disorder of behavioural inhibition — the brain's capacity to pause, evaluate, and select the appropriate response before acting. When inhibition is impaired, the downstream effects cascade through working memory, emotional regulation, time perception, and sustained attention.
But the ADHD brain does not lack attention. It lacks the regulatory system that governs where attention goes. When the environment provides sufficient stimulation — novelty, urgency, physical engagement, immediate consequence — the same brain that cannot sustain focus on a worksheet can lock onto a task with extraordinary intensity. Psychiatrist William Dodson calls this the "interest-based nervous system." Attention flows toward what is compelling and away from what is not, regardless of importance.
Elite gymnastics is, by its nature, compelling. Every skill demands total sensory engagement. A Yurchenko double pike — the vault now officially named the Biles II, the hardest in the women's code of points — involves a round-off onto the springboard, a back handspring onto the vaulting table, and two full rotations in pike position before landing. The entire sequence takes under two seconds. The consequence of inattention is not a missed question on a test. It is a catastrophic landing.
Hyperfocus — the state of deep, sustained engagement that many people with ADHD experience when a task is sufficiently stimulating — occurs most powerfully in goal-oriented situations with immediate feedback. A gymnastics routine is exactly that: a series of discrete, physically demanding goals, each producing instant sensory feedback on execution. For Biles, the gym floor is the one environment where the ADHD brain's demand for intensity is perfectly met by the task.
The cognitive dimensions behind the medals

At the Tokyo Olympics in July 2021, during the team vault final, Simone Biles launched into what was meant to be a two-and-a-half-twist Amanar. Midway through the rotation, something went wrong. She completed only one and a half twists. She stumbled on the landing. She looked, in the words of commentators, "lost in the air."
What Biles experienced is known in gymnastics as the "twisties" — a sudden, terrifying disconnection between the brain's motor plan and the body's execution. The gymnast loses their sense of spatial orientation mid-skill. The internal model that the cerebellum uses to track the body's position stops matching the actual sensory input. The result is not a minor error. It is a complete loss of air sense — and at the heights and speeds Biles operates at, it is genuinely dangerous.
The neuroscience is instructive. Under excessive pressure, athletes may attempt to take conscious control of movements that were previously automatic. This shift from implicit, cerebellar-driven motor execution to explicit, cortically mediated control can actually degrade performance. The prefrontal cortex — the same region that is already under-resourced in ADHD — tries to micromanage a process that the cerebellum handles far more efficiently when left alone.
Stress-induced cortisol compounds the problem. The motor cortex, cerebellum, and spinal cord all carry glucocorticoid receptors. When cortisol floods these structures, the communication between them degrades. The brain's expectation of sensory input and the actual input stop matching. The gymnast is, quite literally, lost.
For someone with ADHD, the vulnerability may be amplified. The prefrontal cortex is already operating with reduced functional connectivity. Emotional dysregulation — the difficulty modulating the intensity of an emotional response — is increasingly recognised as a core feature of ADHD, not a side effect. A 2023 systematic review published in PLOS ONE found that emotional dysregulation should be considered a "fourth core symptom" of ADHD, with adults showing significantly greater use of non-adaptive regulation strategies under stress.
Biles withdrew from the team final, then the individual all-around, then three of four event finals. She returned only for the balance beam, winning bronze. The neuroscience is unambiguous: competing through the twisties at her difficulty level is not a question of mental toughness. It is a question of whether the brain can safely compute the body's position in space.
Biles later connected the crisis to the cumulative weight of trauma — including her experience as a survivor of Larry Nassar's abuse. The pressure of the Games, the isolation of the pandemic, and the unresolved psychological burden converged. The twisties were the visible symptom. The dysregulation was systemic.
How therapy rebuilt the bridge
Biles did not simply return to competition. She rebuilt the psychological infrastructure that competition requires. She began seeing a therapist weekly — a practice she has described as "religious." Before two of her biggest competitions at the 2024 Paris Olympics, she had therapy sessions with her US-based therapist, who was awake in the middle of the night to work with her across time zones.
"Being in a good mental spot, seeing my therapist every Thursday is kind of religious for me," Biles told reporters in Paris. "So that's why I'm here today."
She also set deliberate boundaries — a practice that maps directly onto what masking research describes as the cost of chronic compensation. The withdrawal in Tokyo was the moment the mask came off. The therapy was the process of learning to function without it.
At the Paris 2024 Olympics, Biles won three golds and a silver. She became the most decorated American Olympic gymnast in history with eleven Olympic medals, seven of them gold. She was twenty-seven. She had been through foster care, adoption, abuse, a public mental health crisis, and a two-year absence from competition. And she was, by every statistical measure, the greatest gymnast who had ever lived.

The narrative around ADHD in sport often falls into one of two traps. The "superpower" framing treats ADHD as an underappreciated gift. The "overcame" framing treats it as a pure obstacle. Neither is accurate.
What Biles' career illustrates is that cognitive profiles are not one-dimensional. A mind that struggles with sustained attention in low-stimulation environments can excel in high-stimulation ones. A sensory-motor system that processes proprioceptive information at extraordinary speed can coexist with an emotional regulation system that is more volatile than average. Strengths and vulnerabilities are not opposites. They are different dimensions of the same brain.
